Please fill in your name, address and e-mail address in the following form. Then print it and send it with a cheque for £11.99 (or pro-rata for multiple orders) to our publisher at the address below. If you prefer you can simply print out the blank form and fill it in manually with a pen. If you haven't got a printer then you can just write it all down on a piece of paper (this option may also be useful if your printer has gone into "ink salesman" mode). Providing us with your e-mail address is not obligatory but will enable us to acknowledge your order and inform you when it has been sent. All information will be treated in the strictest confidence.
